Please read this carefully. By proceeding with clinical services through PepLab, you acknowledge that you have read and understood this Telehealth Informed Consent.
What Is Telehealth
Telehealth involves the delivery of healthcare services using electronic communication technologies, including video conferencing, asynchronous messaging, and digital data exchange. Through telehealth, you can consult with a licensed physician without being physically present in the same location.
Benefits of Telehealth
Access to specialized physicians regardless of geographic location
Elimination of travel time and waiting room time
Convenient scheduling consistent with your professional commitments
Ability to consult from any private location
Efficient access to follow-up care and protocol adjustments
Limitations and Risks of Telehealth
Telehealth has limitations compared to in-person care that you should understand:
No physical examination: Your physician cannot perform a physical examination through telehealth. Certain conditions that would be identified through physical examination may not be detectable
Technology limitations: Telehealth depends on functioning technology. Technical failures may interrupt or degrade the quality of consultations
Incomplete medical history: Your telehealth physician may not have access to your complete medical history, prior records, or the records of your other treating physicians
Emergency limitations: Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 immediately
Prescribing limitations: Certain medications cannot be prescribed via telehealth under federal or state law. Your physician will advise you if this applies to any aspect of your protocol
Your Rights and Responsibilities
You have the right to:
Refuse telehealth services and seek care through traditional in-person means at any time
Stop a telehealth consultation at any time
Review information that has been transmitted electronically about you
Request an in-person consultation if your physician determines it is clinically necessary
You are responsible for:
Providing accurate and complete health information
Informing your telehealth physician of all medications, supplements, and treatments you are currently using

Contacting emergency services if you experience a medical emergency
